Your new role

The duties which the individual will be asked to undertake are:

  • Working with CHC Nurses to co-ordinate the booking of Decision Support Tool (DST) meetings with the family, care home and North Lincolnshire Council, confirming appointments and emailing and recording on our system of same.
  • Linking with the CHC Nurse throughout.
  • Working with the PHB Business Co-Ordinator the booking of fit testing, recording outcomes, recording release of PPE to provider and Personal Health Budget Employers etc
  • Other generic administrative tasks.
